We have revised our sample submission and chain-of-custody forms based on feedback from clients and our own internal review. The updated forms include clearer fields for project reference numbers, improved preservation and handling instructions, and better organization of contact information.

The new forms are available through our Client Resources page or by contacting the laboratory directly. Clients currently using older form versions should transition to the new versions at their convenience. We will continue to accept older forms during the transition period.
