Sample Submission Forms
Use the appropriate form to provide sample identification, project instructions, and contact information. The submission form can be filled in on screen or printed and completed in ink.

Shipping & Sample Delivery
Please contact GML before shipping samples. Requirements may vary depending on the project, preservation needs, sample stability, and regulatory considerations.
- Contact the laboratory to discuss the project and obtain a project reference, if applicable.
- Label each sample container clearly and include the appropriate submission documentation.
- Package samples securely to prevent breakage or leakage during transport.
- Use the shipping instructions provided by the laboratory representative.
Turnaround Times
Standard turnaround is 1 week from sample receipt. Rush and expedited service may be available for an additional charge; timing for complex methods, large sample batches, and custom projects may vary with the current laboratory schedule. Please discuss timing requirements with us before submitting time-sensitive samples.
